Care Coordinators - Recruitment Support
Care coordinators help to co-ordinate and navigate care across the health and care system, helping people make the right connections, with the right teams at the right time.
They can support people to become more active in their own health and care and are skilled in assessing people’s changing needs. Care coordinators are effective in bringing together multidisciplinary teams to support people’s complex health and care needs.
What is a care co-ordinator?
Care co-ordinators work with people to build trusting relationships and listen to what matters to them. They work with a range of people, particularly those with long-term conditions, multiple long-term conditions, and people living with or at risk of frailty. They help people co-ordinate and navigate their care across the health and care system and can support people to become more active in their own health and care.
Due to the complexity of navigating the health and care system, some people may benefit from support to understand and make sense of this ‘web’ of interactions and which team/professional to get support from, including nurses, counsellors, therapist, activity workers, GPs, surgeons, care worker and managers. Care co-ordinators help people make the right connections, with the right teams at the right time.
Care co-ordinators are at the centre of a person’s care. They walk alongside the person and are uniquely placed to see the bigger picture, from clinical care to how their social interactions support their wellbeing.
The Care Coordinator role within Personalised Care
In general practice a care coordinator will:
work closely with patients to support their personalised care requirements
develop person-centred service plan (PCSP) with patients
understand local services and enable patients to access these appropriately
support people to understand their level of knowledge, skills and confidence when engaging with their health and wellbeing
raise awareness of shared decision making
