General Practitioners play an important role in looking after patients in the community. They would typically, promote, prevent, initiate treatment and refer patients into secondary care. GP's look after patients with chronic illnesses with an aim to keep them in their own home and ensure that they are as well as possible. This is where GPs can come to find out information about what offers, funding and development opportunities the Training Hub currently has on offer.

Diadem tool for GPs – GPs diagnosing advanced dementia in care homes
There are some Herts care home residents with advanced dementia whom have never had a formal diagnosis.
In these cases, a referral to memory services is rarely desirable. It is likely to be distressing for the individual and is usually unnecessary. These referrals are also putting added pressure onto the memory clinic.
People with advanced dementia, their families and staff caring for them, still benefit from a formal diagnosis. It enables access to appropriate care to meet individual needs and prompts staff to consider MCA and DOLs issues where appropriate.

Then add clinical coding to the patient record.
